What material was involved? One primer (450g cast booster and an electronic detonator)
What happened?
The truck operator was loading holes on his right-hand line, and the
bench crew had made up primers in anticipation on the left-side line.
The bulk truck became stuck on the bench’s loose material (sand)
and had to be towed out by another piece of equipment.
The truck slid slightly sidewise, during the towing process and ran
over one assembled primer.
The towing process was interrupted and was reinitiated after the
remaining primers were disassembled and relocated to a safe
distance.
Why did it happen – theory?
The key point is that primers should not be made in anticipation.
Contributing factors:
– Poor bench conditions (approx. 6% cross slope and loose material)
– Lack of official spotter on the offside of the truck during the
recovery.
What was the impact? No consequences – Categorized as high potential near miss
